These data will serve as preliminary data to support a joint NIH application between the School of Health and Rehabilitation Sciences and the Department of Family Medicine focused in the Patient Centered Medical Neighborhood.

Your participation is voluntary and no personal information will be collected during the survey. The data will be used in aggregate to describe the current of care provided in the primary care setting. At the end of the survey, you be directed to a link to a survey to be entered for a chance to win one of six $50 Amazon gift cards or one of two $100 Amazon gift cards.
